The Enhance Health Angel Fund
Enhance Health contributes $1 from every new submitted application to the Angel Fund, which supports employees facing financial hardships due to catastrophic events.
Eligibility Criteria
Employees must meet the following requirements to qualify as an Angel Fund recipient:
- Employment Status: Must be a current employee of Enhance Health
- Financial Hardship: Must be facing financial difficulties due to a catastrophic event (e.g. natural disaster, severe illness or death of a family member)
- Application Submission: Must submit a completed application form to the Enhance Angel Fund Committee
Application Process
- Form Submission: Complete and submit the form below
- Review Period: The Enhance Angel Fund Committee reviews all submissions within 72 hours
- Assistance Determination: The Committee determines the amount of assistance to be awarded based on the severity of hardship
